• You have the option to schedule your initial evaluation with either Dr. Kenneth Liegner or Erica Glazer, FNP. Following your initial in person visit, we offer a hybrid approach where patients can choose between in-person and telemedicine appointments.

• Please bring a set of pertinent medical records to be uploaded to your chart. You should also keep your own set of records. Pertinent records include your most recent labs, any and all prior testing for tick-borne illness, and any other labs or imaging studies relevant to your current condition.

• There will be a reservation fee of $100 applied to the visit. This will be refundable if notice of cancellation is provided with at least 2 of our business days' notice.

• Initial evaluations often take 2 hours, but may take longer. Please plan accordingly.

• The practice is fee-for-service. A HCFA 1500 form will be provided to patients which they can submit to their insurer for reimbursement to them.

• For patients who prefer managing their care digitally, we offer access to our secure Cerbo patient portal for convenient communication and record tracking.

• Initial evaluations and first follow-up visits are recorded for accuracy and for transcription purposes. If you are not comfortable with this, we suggest you seek care elsewhere.
